1. Request Health Care Records
Under Section 181.105 of the Texas Health and Safety Code, you have the right to access your health care records.
- Submission: Requests must be made in writing. Please submit your request via email to admin@risewellgroup.com
- Response Time: By law, licensees must provide copies of your records within 15 business days of receiving your written request.
- Fees: A reasonable fee for copying and mailing may apply, but you cannot be charged a retrieval fee.
