Rise Wellness Group

Fees and Billing

Insurance

We do not accept insurance in consideration for payment. Services provided by therapists of Rise Wellness Group are considered "out of pocket" or "cash pay". All payments are due in full at the time of each session. Upon request, we will provide detailed billing that you may submit to your insurance for possible out-of-network benefits.

Out of Network Benefits

In order to verify whether services provided by Rise Wellness Group can be reimbursed to you, we recommend that you contact your insurance company. Individual insurance companies have different out of network policies and may or may not cover incurred expenses. Rise Wellness Group is unable to submit claims directly on your behalf nor verify your benefits with any insurance plans.

Payments

We accept HSA and FSA cards, cash, check and all major credit cards for payment. Payment information must be on file before first appointment. A $25 deposit will be collected at the time of scheduling your first session and will be credited towards your initial appointment fee. Reduced fee services are available on a limited basis. A $3 service charge is added to all credit card charges.

Cancellation Policy
We require notice of cancellation within 24 hours of your appointment time via phone call or email. You may leave a message on the main voicemail if after hours or during the weekend. Full session fee will be charged for failure to notify and cancel appointments within 24 hours of your scheduled time. Additional consideration will be given on an emergency basis and should be discussed with your therapists.

Good Faith Estimate
Under Section 2799B-6 of the Public Health Service Act, health care providers and health care facilities are required to inform individuals who are not enrolled in a plan or coverage or a Federal health care program, or not seeking to file a claim with their plan or coverage both orally and in writing of their ability, upon request or at the time of scheduling health care items and services, to receive a “Good Faith Estimate” of expected charges.

You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost
Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.

You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.

Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.

If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.

Make sure to save a copy or picture of your Good Faith Estimate. For questions or more information about your right to a Good Faith Estimate, visit http://www.cms.gov/nosurprises
